What OSHA asks of you
Regulator logs and the deadline clocks Coming in Q1 2027
What goes on the record
A new work-related injury or illness that results in death, days away from work, restricted work or transfer to another job, medical treatment beyond first aid, loss of consciousness, or a significant injury or illness diagnosed by a physician or other licensed health care professional.
How it is kept
- Within 7 days of learning of it, on the OSHA 300 log.
- One record per establishment.
- Names withheld on privacy cases.
- Kept for 5 years.
- Injury rates per 200,000 hours worked.
| What happened | Who is told | When | How |
|---|---|---|---|
| A work-related fatality, reported when the death happens within 30 days of the incident. | OSHA | Within 8 hours of learning of it | by phone, online |
| A work-related in-patient hospitalisation, amputation or loss of an eye, within 24 hours of the incident. | OSHA | Within 24 hours of learning of it | by phone, online |
The forms, filled in from the report
- OSHA Form 300: Log of Work-Related Injuries and Illnesses, with the OSHA Injury Tracking Application case data file
- OSHA Form 301: Injury and Illness Incident Report
- OSHA Form 300A: Summary of Work-Related Injuries and Illnesses, with the OSHA Injury Tracking Application 300A summary file
The report is made once. Every field the form shares with it is filled in; you add what only you know, check it and send it yourself.
Dates in the year
- Post the 300A summary where notices to employees are usually posted, 1 February to 30 April.
- Submit the year’s data electronically through OSHA’s Injury Tracking Application, by 2 March.
- Keep the 300 log, the 300A summary and the 301 reports for five years after the end of the year they cover.
A case, judged
A worker cuts their hand and has first aid only: Not recordable. Nothing to report to OSHA.
If it keeps them off work for 8 days: OSHA 300 log: Days away from work (H), by Oct 12, 2026. Nothing to report to OSHA.
Athena proposes the answer and shows the rule it used. A person decides, and only a person sends anything to OSHA.
