What goes on the record
Every notifiable incident is recorded and kept; every work injury goes in the register of injuries.
How it is kept
- One record per employer.
- Kept for 5 years.
- Injury rates per 1,000,000 hours worked.
| What happened | Who is told | When | How |
|---|---|---|---|
| A notifiable incident (a death, a serious injury or illness, or a dangerous incident): the regulator told immediately by the fastest means, written notice within 48 hours if it asks, and the site kept as it is. Keep the site as it is. | the WHS regulator | Immediately | by phone, online |
Dates in the year
- Keep a record of each notifiable incident for at least five years.
A case, judged
A worker cuts their hand and has first aid only: Injury. Nothing to report to the WHS regulator.
If it keeps them off work for 8 days: Register of injuries: Lost time injury. Nothing to report to the WHS regulator.
Athena proposes the answer and shows the rule it used. A person decides, and only a person sends anything to the WHS regulator.

Where these rules come from
Drafted from the public sources below, read on 26 October 2026.
Not yet reviewed by a qualified person in Australia: until one has, every rule is marked to be checked, in the product as here. Each rule has a version, and a case keeps the version it was judged under.
Run the WHS regulator’s rules on your own sites.
