What HSA asks of you
Regulator logs and the deadline clocks Coming in Q1 2027
What goes on the record
Every accident that keeps an employee from their normal work for more than three consecutive days, not counting the day of the accident, every fatal accident, and every dangerous occurrence.
How it is kept
- One record per employer.
- Kept for 10 years.
- Injury rates per 1,000,000 hours worked.
| What happened | Who is told | When | How |
|---|---|---|---|
| A fatal accident: the HSA or the Garda told immediately, then the IR1 form. | HSA | Immediately | by phone, online |
| An employee unable to do their normal work for more than three consecutive days, not counting the day of the accident. | HSA | Within 10 working days | online |
| A dangerous occurrence listed in Schedule 13. | HSA | Within 10 working days | online |
The forms, filled in from the report
- IR1: report of an accident
- IR3: report of a dangerous occurrence
The report is made once. Every field the form shares with it is filled in; you add what only you know, check it and send it yourself.
Dates in the year
- Keep a record of every reportable accident and dangerous occurrence for ten years.
A case, judged
A worker cuts their hand and has first aid only: Accident register only. Nothing to report to HSA.
If it keeps them off work for 8 days: Accident register: More than three days unable to work. HSA, by 19 Oct 2026, on the IR1: report of an accident.
Athena proposes the answer and shows the rule it used. A person decides, and only a person sends anything to HSA.
In Ireland, it is called

Where these rules come from
Drafted from the public sources below, read on 26 October 2026.
- Safety, Health and Welfare at Work (General Application) Regulations 2016 (S.I. No. 36 of 2016, regulation 74)
- HSA: accident and dangerous occurrence reporting
- HSA: report a dangerous occurrence
Not yet reviewed by a qualified person in Ireland: until one has, every rule is marked to be checked, in the product as here. Each rule has a version, and a case keeps the version it was judged under.
